Forms for Public Access Producers and Programmers
	Community Programming Agreement	Submit along with copies of shows to be shown on Public Access 24.
	Series Application	Submit when requesting a regular day and time for a series of shows.
	Video-on-Demand Application	Submit when requesting a show to be encoded and available for Video-on-Demand (requires a fee).
	Project Proposal	Submit when starting a new individual video or series.
	AMP Voting Membership Application	Monterey and Marina residents only. Required to participate in Board elections.
	Underwriter (Sponsor) Agreement	Must be submitted with any programming for Channel 24 mentioning underwriters.
	Volunteer and Intern Agreement	Submit if interested in an internship or becoming an on-going volunteer with AMP.
